The run record's narrative body, verbatim. This is where the run accounts for its own judgement calls: every borderline drop and judged-not-relevant item with its reason, dedup decisions, single-source items and their carve-outs, contradictions, and per-source coverage gaps, so nothing the run considered disappears silently.

Verification & coverage notes
Cleanup wave 02 of the one-time store cleanup program (prompts/cleanup-coordinator.md), an operator-directed partial audit with a partial claim. It ran no coverage re-sweep, so it does not move the weekly audit's window anchor. Report: docs/audits/2026-10-21-cleanup-wave-02.md. Tracker: docs/cleanup/STATUS.md.
Scope. Rows W1-B (57 imprecisions from the 2026-10-18 truth passes), W1-C (5 clean legacy entries), W1-E (34 carried residuals and advisories), W1-F (the 8 reference-branch entry hunks) and W1-H (the Drupal page-check lead): 85 entries in six batches, one repair worker each (work/2026-10-21T0800Z-audit/batch-<n>.yaml, repair-brief.md, repair-<n>.md). No truth pass ran, since these rows carry fresh findings (§ C2).
Outcome. 72 entries carry one record from this fire: 61 improvements (18 internal) and 11 corrections (1 internal), no update. 13 needed none (already fixed by an earlier audit, or declined with a reason the report lists). The 18 legacy entries of W1-B and W1-C left the legacy queue, 406 to 388 pending. At the gate the main agent amended the LiteSpeed record summary and two sentences (LiteSpeed, Ivanti) so the IOC scan stops reading version numbers as IPv4 addresses. Registry: the FTAPI incident summary corrected, four product records from tools/sync_products.py. cves_seen: CVE-2026-92438 added, 33 last_seen moves, four titles aligned with their entries.
Standalone merges during the wave: 731706f (W7-14, the claim ledger lists sentences edited inside earlier sections) and 1f8d69e (W7-15, a bot-challenge page counts as an unread page in the page checks). The wave's plan and batch files went to main as ea6e509 while the workers ran. By operator directive during the wave, the program moved to prompt v4.23 (b511bbe): cleanup waves run in parallel on lanes the tracker keeps disjoint, this wave's claim gained its wave=02 tag, and its rows were marked in flight on main. A lane fix followed (17506d0). This wave executed under v4.22 and records that version, so the run-scoped gate notes that it trails the CHANGELOG head.
Warnings. The run-scoped gate carries one quote-literal WARN on Pixel CVE-2026-58704: the container receives an older copy of Google's bulletin without its 2026-09-30 note, which repair-4 read verbatim through WebFetch (work/2026-10-21T0800Z-audit/repair-4/src/e11-pixel-webfetch.txt). It is a transport gap, not a quote defect, and the verifier checks the quote by hand.
Budget. 19 of 20 spawns (work/2026-10-21T0800Z-audit/budget.tsv): six repair workers and 13 verifier spawns, of which three were continuations after the account-wide session limit stopped a pass (iteration 6's two slices at about 12:25Z, iteration 7 at about 16:10Z). About 9.13 M sub-agent tokens in all (repairs 2.23 M, verifier 6.89 M), not counting the three stopped legs, whose usage was not reported. The limit was shared with waves 03 to 05, which started in parallel under v4.23 while this wave ran.
Verification. Eight passes of cti-verification, the loop's cap. Iteration 1 (two slices, all 541 changed claims) found 10 truth and 1 editorial findings. Post-fix passes 2 to 4 found 3, 4 and 2 (truth plus editorial), each a citation bound to a page that carries only part of the clause, or a stale count in this wave's own files. Iteration 5 returned CLEAN. The confirmation pass, iteration 6 (two slices, all 552 claims, both resumed on Opus 5.5 after the limit), refuted it with 6 truth findings, five of them on this wave's tracker and report after waves 03 to 05 changed the plan on main. Iterations 7 and 8 found 1 and 2 truth findings, all in the report and the carried W1-I file. Every finding was fixed or declined with a reason in the iteration records. The loop reached the cap and publishes on the fail-open rule with residual 2: iteration 8's two count fixes, applied after the pass and not re-verified.
