Verification & coverage notes
Cleanup wave 03 of the one-time store cleanup program (prompts/cleanup-coordinator.md, v4.23), lane A, an operator-directed partial audit with a partial ... wave=03 claim. It ran beside waves 02, 04 and 05 and ran no coverage re-sweep, so it does not move the weekly audit's window anchor. Report: docs/audits/2026-10-21-cleanup-wave-03.md. Tracker: docs/cleanup/STATUS.md.
Scope. Rows W4-01, W4-03, W4-04, W4-05 (duplicates) and W1-D (33 entries with the 2026-10-18 audit's D1 to D3 and L verdicts, 366 listed defects, read before the 2026-10-11 audit's records landed). The duplicate pairs were decided first, then the six W4 members got a whole-entry truth pass (truth-T1), and five repair workers composed the records.
Outcome. 23 entries carry a record from this fire: 2 updates (YellowKey, Nightmare Eclipse, both fixed on 2026-06-09), 6 corrections (one internal) and 15 improvements (one internal). 12 W1-D entries needed none, the 2026-10-11 audit having fixed every listed defect. One fold: the legacy "Large-scale ClickFix campaign" entry into the Ghost CMS CVE-2026-26980 ClickFix entry, carrying only the twin's verified facts. Links: Nightmare Eclipse to YellowKey, Adobe APSB26-120 to APSB26-114, Dirty Frag to Copy Fail, Catalyst SD-WAN CVE-2026-20262 to CVE-2026-20245. The Pwn2Own Berlin roundup was rewritten from ZDI's pages and refiled as routine research. Four legacy entries left the queue (384 pending after merging wave 02, which reviewed 18). The W4-05 scan found four more twin pairs on rows other waves hold, and a fifth (the 2026-05-19 MiniPlasma entry) surfaced in repair. Verifier iteration 5 found that the Fragnesia entry should list Dirty Frag. All six are new tracker rows (W4-06 to W4-11), none edited here.
State. cves_seen: last_seen moved for seven CVEs and four stale titles refreshed. Registry: two summaries corrected (campaign:nightmare-eclipse-microsoft-dcu-threat-greenplasma-miniplasmaaac, incident:pwn2own-berlin-2026). tools/sync_products.py added five product records and moved first_seen earlier on eight others that the rewritten Pwn2Own roundup, YellowKey and Nightmare Eclipse entries now name (Cursor, LiteLLM, Microsoft Defender Antivirus, Microsoft Edge, Microsoft SharePoint, Microsoft Windows Server, OpenAI Codex, VMware ESXi). No relation lost its source entry.
Process. Waves 03 and 05 computed the same run id in the same minute. Wave 05's claim landed first, so this wave withdrew its claim and re-claimed under this id. Truth pass T1 ran as a claude sub-agent with the cti-verification definition loaded by instruction, on sonnet, because the session started outside the repository and the named type was not registered yet. Every Phase 5.7 iteration used the named type. The account session limit cut repair-5, verifier iteration 1 and verifier iteration 3 once each. Under § C8 none was retried in a loop. Each was resumed once after the reset, and all three continuations count in the budget.
Budget. 17 of 20 spawns (work/2026-10-21T1149Z-audit/budget.tsv): one truth pass, five repair workers, eight verifier passes and three continuations, about 6.88 M sub-agent tokens (truth pass 0.47 M, repairs 1.68 M, verifier 4.74 M).
Verification. Eight passes of cti-verification, the cap. Iterations 4 and 6 returned CLEAN and the confirmation passes after them (iterations 5 and 7) refuted both on citation-precision points. Iteration 8, a full-scope pass at the cap, returned NEEDS_FIXES with three truth and one editorial finding. All four were fixed after the pass and published on the fail-open rule without re-verification, so the residual is 4. Every pass answered every claim in its scope.
